CCCW Transition Teams
Transition Planning Committee
Members of the Transition Planning Committee included Directors and County Board Supervisors from all three counties. March 26, 2007 was the final meeting of the Transition Planning Committee as they have met their charge of creating and gaining each county's approval of an intergovernmental agreement for CCCW. With this agreement, Marathon, Wood and Portage counties have created a separate legal entity as authorized through Section 66.0301 of the Wisconsin State Statutes for the purpose of coordinating and providing long term support services for residents of the tri-county service area.
Access Workgroup
The Access Workgroup, made up of supervisory staff from Aging and Disability Resource Centers, Long Term Support Units, and Economic Support Units in Marathon, Portage and Wood Counties, created detailed plans for assisting individuals to access the Family Care benefit. This workgroup submitted three flow charts which detail the processes to be completed in: (1) transitioning residents currently receiving waiver services to Family Care; (2) offering the opportunity to enroll in Family Care to individuals on waiting lists; and (3) assisting new consumers in the entire region to enroll in Community Care of Central Wisconsin.
Education Workgroup
Long term care supervisors and staff from Marathon, Portage, and Wood Counties are working to keep individuals informed during the planning process and rollout of Family Care in Marathon and Wood Counties. In July of 2006, this group sponsored a Staff Education Day for all tri-county staff that will be affected by the expansion. This group continues to develop and recommend strategies for introducing Family Care to current long term care consumers, individuals on waiting lists, staff, and communities at large.
Provider Education Workgroup
The Provider Education Workgroup, a subsidiary of the larger Education Workgroup, is charged with the task of educating providers during the expansion process. A Provider Education Day was sponsored in September of 2006 and quarterly newsletters are currently being used to provide information and updates to long term care providers in Central Wisconsin.
