Community Care of Portage County is pleased to be a partner in the Family Care expansion planning that is taking place in Central Wisconsin. As the Provider Network Manager for Portage County, I want to share my enthusiasm for the expansion and my commitment to making this a positive transition for current Members of CCPC and future members of Community Care of Central Wisconsin (CCCW). We realize that a successful transition will happen only if we continue to build and support strong partnerships with our providers.

During the upcoming months and into Family Care implementation in Marathon and Wood Counties, Community Care of Central Wisconsin will be dedicated to expanding our provider network, bringing added choice to our consumers and increased opportunities to our providers. Our Network team will target the following critical areas during the transition: expansion timelines and rate setting; capacity and quality of the current and expanded network; continued provider education about Family Care and the role of the network; and provider training.

In anticipation of becoming a multi-county agency, we are in the process of creating an electronic Provider Directory for staff and consumer use. Please help us by completing the questionnaire and returning either by email or U.S. mail. If you are not currently in the CCPC provider network, but anticipate becoming part of the CCCW Provider Network with expansion, we will need this data to ensure that our consumers, social workers, nurses and administration have accurate and complete information about the services you provide and the target groups you serve.
