There are two organizational components of Family Care:

Aging and Disability Resource Centers


Aging and Disability Centers (ADRCs) are "one-stop-shops" where you can get information and advice about available long term care resources. In this new and simplified system for accessing information and assistance about community services and long term care, Resource Centers serve the general public through:

  • Outreach
  • Information and assistance
  • Youth transitional services
  • Options Counseling
  • Pre-admission consultation (PAC)
  • Early intervention/prevention
  • Eligibility determination and enrollment in a care management organization for those seeking publicly funded long term care services (such as the Family Care benefit)

The Aging and Disability resources centers of Wisconsin are welcoming and convenient places for the general public to get information about long term care. They offer a single point of entry for publicly funded long term care services. Services are provided through the telephone, walk-in office visits, or visits to an individual's home.

Managed Care Organizations

Managed Care Organizations (MCOs) deliver the Family Care benefit to those who are eligible. Funding is streamlined into one flexible long-term care benefit that is provided according to individual circumstances, needs, and preferences. MCOs work to continuously improve the quality of care and services of their members. By developing a comprehensive network of long-term care services and support, MCOs are able to provide the choice, quality and cost-effectiveness that are primary goals of the Family Care model.

The Managed Care Organization provides service coordination through in interdisciplinary team approach that focuses on:

  • Outcomes-based care planning
  • Prevention and wellness
  • Coordination between long term care and acute/primary health care
  • Quality assurance and improvement

Services and supports included in the Family Care Benefit:

  • Adaptive aids, communication aids, medical supplies, home modifications
  • Home health, nursing services, therapies
  • Personal care, supportive home care
  • Transportation
  • Residential services, nursing facility, home modifications
  • Supportive employment
  • Meals: home delivered and congregate
  • Respite care, adult day care, day services, emergency response systems
  • Case management
