Community Care of Central Wisconsin, or CCCW, is a managed long term care organization certified by the State of Wisconsin to deliver the family care benefit to eligible residents of Marathon, Portage and Wood counties. CCCW offices are located in Stevens Point, Wausau, Marshfield and Wisconsin Rapids, with Stevens Point being the administrative center.
CCCW provides each member with an interdisciplinary service coordination team that works with that individual to develop a member centered care plan. This care plan is focused on identifying and supporting individual outcomes that are important to the member through the use of services and goods available through the family care benefit package.
Membership in Community Care of Central Wisconsin is determined through an eligibility and enrollment process conducted by the local aging and disability resource center. Seniors and adults with either physical or developmental disabilities are eligible for family care consideration.
Services and goods are provided through a broad network of providers under contract with CCCW. Over 30 different forms of long term care are included in the family care benefit package and delivered by the provider network.
Community Care of Central Wisconsin is formed as a long term care district operating under Wisconsin State Statutory language created in 2007. It is governed by a nine member board of directors representing the interests of residents residing in the three counties served. CCCW has succeeded Community Care of Portage County as the Managed Care Organization in central Wisconsin. Community Care of Portage County served Portage County residents as one of five Wisconsin family care pilot sites from 2000 to 2008, successfully serving nearly 2000 Portage County residents during that period of time.
