Watch CEO Video Hello and welcome to the Community Care of Central Wisconsin (CCCW) website! My name is Jim Canales and I have the pleasure and honor of serving as the inaugural Chief Executive Officer for CCCW.
CCCW has become the Managed Care Organization (MCO) operating the State of Wisconsin Family Care program in the counties of Marathon, Portage, and Wood. Family Care provides a package of long term care benefits that support the identified outcomes of our participants. These participants, known as members, must meet functional and financial requirements of the Family Care program and either be elderly or be an adult with a physical or developmental or disability, in order to participate.
I have worked in the field of long term care since 1977, and most recently was employed by Portage County as their Director of Community Care of Portage County. CCPC was one the original five county pilot sites for Family Care from 2000-2008.
I have been married to Nancy for the past 22 years, have two daughters in college, and own a 4 pound Yorkshire Terrier and an 75 pound Golden Doodle. We reside in the Stevens Point area on a quarter acre lot that boasts a multitude of trees, shrubs and perennials that I tend to when I am not working, or am at the best volunteer job in the world...hosting/producing jazz, blues, and acoustic radio shows at the UWSP Campus Radio Station, 90FM.
My initial responsibilities as CEO with Community Care of Central Wisconsin include:

Creation of the infrastructure and systems needed for CCCW to operate.

Hiring of qualified, expert, and enthusiastic staff.

Assuring that CCCW meets all standards and requirements associated with being a Managed Care Organization.

Assuring that CCCW met all expansion timeframes as set by the CCCW Board of Directors.

Working with the CCCW Board of Directors to develop and carry out policies and procedures necessary for CCCW to operate effectively and efficiently.

Working with CCCW staff and Board to develop organizational values and an organizational culture that promote collaboration, respect, and responsiveness to all stakeholders
I am happy to say that in the last 10 months, CCCW has met the expansion timeframes as set by the Board, and is now state certified and operating in the Counties of Marathon, Portage, and Wood. Over 2600 members are enrolled in CCCW today, and are being served by qualified and dedicated staff operating from four offices located in our region.
CCCW has begun to work with the Board to develop policies and procedures necessary for effective service delivery to our members, and to identify and develop over time the organizational values that will define our work culture and how we are perceived by the community and ourselves.
Many opportunies lie ahead for CCCW, our members, and our communities. They include the elimination of all wait lists for our adult individuals with long term care needs; the development of new and expanded provider options for our membership; and an opportunity to develop additonal quality services that support the outcomes of our members in a cost effective manner.
