Community Care of Central Wisconsin, or CCCW, is the Managed Care Organization contracted by the State of Wisconsin to provide eligible residents of Marathon, Portage and Wood counties the benefits, services and goods associated with the Family Care program.

The mission of Community Care of Central Wisconsin is to identify and support the unique health and long term care outcomes of seniors and adults with developmental and physical disabilities residing in Wisconsin counties of Marathon, Portage and Wood, by providing high quality and individualized long term care services available through the Wisconsin Family Care program. |
